根据《传染病法》(第137章)和《2021年传染病(COVID-19疫苗接种)条例》，这些信息是强制性的。只有年满18岁且持有新加坡有效证件的的成年人可以申请，如果您在新加坡只是持有护照的话，不符合接种的条件 。

注意事项:。

1. 请携带您的新加坡证件(NRIC/FIN)。

2.请在您选定的日期和时间到达。不要早到或晚到10分钟。
